At , our Medical Records (or Health Information Management) Departments are committed to providing the highest quality of service to our patients, family members, physicians, hospital staff and other healthcare information users. Our role is to manage and provide access to patient health information while complying with all internal and external regulations and standards.

Baptist Medical Center includes five major hospital listed below. Photocopies of your health records are available upon request from the Health Information Management Department at the hospital where you were a patient. If you have any medical record questions that are not addressed here, please call the specific facility's HIM department for further assistance.

Who can obtain copies of medical records?

Answer:  

Only the patient or his/her legally authorized representative can sign the authorization and determine who can obtain copies of the medical records. Proper identification is required prior to the release of the confidential health information.

How do I obtain copies of my medical records?

Answer:  

All requests for the release of medical records should be submitted through the use of the (click on link to download file) Baptist Medical Center Authorization to Release or Obtain Medical Information Form. The form should be dated and signed by the patient or the patient's legally authorized representative. In case of a minor, the parent or guardian should sign the authorization. Once the form is completed, you may submit your request by mail, fax or in person.

Please note that if you are under psychiatric care, your request will need to be reviewed by your psychiatrist prior to the release of your health records.

NOTE:

  •  Request for billing statement copies should be directed to Patient Financial Services (click here for the contact information)
  • How to obtain Radiology films/scan?

    Answer:  

    Medical Records department can only complete requests for radiology typed reports. To request for X-ray, MRI or other radiology films/CDs, you must submit a signed and dated request authorization form to the Department of Radiology and Medical Imaging.

    How to obtain mammograms in CDs?

    Answer:  

    Medical Records department can only provide a copy of the mammography typed report. To receive your mammography images in CD, you must submit a signed and dated request authorization form.

    NOTE:
    To expedite the request for you mammogram, please call the department and leave a detailed message by following the instructions in the telephone prompt.

    Is there a fee for copies of my medical records?

    Answer:  

    Health records released for continuing care to another healthcare institution or care giver are free of charge. For personal use there is a dollar per page charge. For details, please contact the HIM department (click here for HIM contact numbers)

    Do I need to pay in advance?

    Answer:  

    Payment is only required in advance from a patient if the cost exceeds one hundred dollars ($100.00).

    How long does it take to get copies of my medical records?

    Answer:  

    Copies of the medical records are usually available within 3-5 business days from the time the request is received.

    Am I allowed to amend or change my medical records?

    Answer:  

    Your Protected Health Information (PHI) can be amended only when a proper written request is (click here to download Amendment Form) submitted to the Corporate Compliance department (click here for Corporate Compliance contact numbers).

    How do I obtain copies of Birth Certificate/Death Certificate?

    Answer:  

    To obtain a copy of Birth or Death Certificate, please contact the Vital Statistics Office at 904.359.6900 or visit their main office at 900 University Blvd.N. in Jacksonville. They are open from 8 a.m. to 4:45 p.m., Monday-Friday.
